Measuring Outcomes

Each and every resident of Uplands Rehabilitation Centre undergoes continual assessment and their care plans are fluid in response to this.

The professional team consists of Nursing, Physiotherapists, Occupational Therapists, health and social care workers, working alongside those on the periphery such as GPs, Social Workers, Community Nurses, to name but a few. This approach enables Uplands to provide an hollistic rehabilitation and re-ablement programme which truly factors in all aspects of body and mind. This system has proved to have truly dramatic, enprecedented even, results.

We set individual goals for each client which encompasses comprehensive individual and family involvement using our full multi-disciplinary team and these are dictated by the agreed pathway of care. Although all our programmes are goal-directed, we will always assist clients to set, track and achieve recovery milestones. Goals are always:

  • A – Achieveable

  • R – Realistic

  • T – within a Timeframe
